Identification of Capnocytophaga species from oral cavity of healthy individuals and patients with chronic periodontitis using phenotypic tests

Background: The role of Capnocytophaga species in oral health and disease is not well studied, and there are no reports from India about their prevalence in the oral cavity. Few attempts have been made to identify all seven cultivable species of Capnocytophaga from gingival pocket. The aim of this study was to detect the prevalence of Capnocytophaga species in healthy individuals, gingivitis, and periodontitis using phenotypic tests.

Materials and Methods: A total of 150 adult subjects between the age ranges of 20-55 years were included in the study comprised of 50 each of subjects with gingivitis, periodontitis, and healthy individuals. Subgingival plaque was collected and cultured on blood agar, TBBP, and Dentaid media. Species identification was done by performing biochemical tests and hydrolysis tests.

Results: Among 150 samples, 28 (18.67%) yielded Capnocytophaga species. The prevalence of Capnocytophaga species was statistically analyzed using Chi-square test, Mann-Whitney U-test, and Fisher’s exact test. The prevalence was higher in healthy individuals (30%), compared to gingivitis (14%) and periodontitis (12%). The prevalence of Capnocytophaga ochracea, Capnocytophaga gingivalis, and Capnocytophaga granulosa was more in healthy individuals than in gingivitis and periodontitis.

Conclusion: We conclude that Capnocytophaga is more frequent in healthy human mouth than in diseased individuals. There is a need to further study both sub- and supra-gingival plaques for the presence of this organism.

Technological advances and discoveries in instruments and materials have made it possible to achieve treatment objectives that once were considered unattainable in the field of dentistry. The introduction of microscopes into endodontics in the early nineties brought on a renaissance in endodontics that led to new and exciting discoveries and the blossoming of new ideas and techniques. The microscope proved to be an invaluable instrument, allowing endodontists to render treatment for problems which were previously thought to be impossible to treat. This review article provides a comprehensive review about the applications of microscopes in endodontics.

The imbalance between bacterial virulence and host response leads to the destruction of tooth and tooth-supporting structures, leading to most commonly occurring chronic periodontitis and rare, but not uncommon aggressive periodontitis and the dilemma between diagnosis, prognosis, and treatment planning of these two entities have always been a challenge to clinician at least in few situations. Hence, this article aims at reviewing the shared characteristics (similarities) and unique characteristics (dissimilarities) of chronic and aggressive forms.

Two most common conditions that can independently lead to tooth mortality are periodontal and pulpal diseases. To maintain the natural dentition and also to restore the lost periodontium are the goal of endodontic and periodontal therapy. There are several reviews in literature where authors have given wide information about the etiology, pathogenesis, classification, diagnosis, and treatment planning, and lot of literature about the recent advances pertaining to aspects of endo-perio lesions are also available, but reviews compiling all major aspects are relatively less and this review aims to brief the etiology, various classifications, diagnosis, and management of endodontic-periodontic lesions. Successful treatment of periodontic-endodontic lesions depends on all contributing lesions being treated. Lesions with combined etiology will require both endodontic and periodontal therapy. Endodontic therapy should be completed first. The clinical experience and acumen of the dentist must be of the highest degree to enable choosing the procedure that will bring healing with the appropriate intervention.

Orofacial pain is the big burden over the society and one of the factors which leads to high morbidity and psychological stress. Orofacial pain has different origin and etiology which makes the diagnosis challenging. It also involves disorder which overlaps and makes the diagnosis difficult, but the recent advancement and the multidisciplinary approach help in the management of the orofacial pain and may limit the negative impact of the pain. This paper primarily focused on the diagnosis and management of orofacial pain.

Interspecies communication in oral biofilm

Oral biofilms are composed of >700 bacterial strains in a matrix of salivary glycoprotein and extracellular polysaccharides which display extensive interactions while forming organized structures in a biofilm to carry out physiological functions and induce pathogenesis in microbes. Interactions among oral bacteria in humans are essential to the development and maturation of the plaque. These interactions can occur at several levels which include physical contact, metabolic exchange, small-signal molecule-mediated communication, and exchange of genetic material. These levels of interspecies interaction benefit the microorganism by providing a broader habitat range, effective metabolism, increasing the resistance to host defense, and enhancing their virulence. This generally has a deleterious effect on the host and is attributed to many chronic infections which possess a curative challenge. This review summarizes the interaction between different microbial species present within a biofilm and how they communicate with each other.

Taurodontism, particularly in the primary dentition, is a rare entity. The anatomic configuration provides a challenge to the pediatric dentist in terms of management, particularly endodontic management. Hurdles are faced by the clinician from the phase of access opening till obturation. The following case report tries to add to the existing literature in terms of quicker management strategies when faced with such a clinical scenario.

Apical migration of the marginal gingiva beyond the cementoenamel junction causing exposure of the root surface is called gingival recession. This mucogingival deformity presents a significant esthetic problem. As sequelae, it could lead to pain due to the dentinal exposure or caries of the root. Several approaches and techniques have been advocated for root coverage with predictable success. Here, we present a case report, in which root coverage has been successfully achieved in gingival recession of Class I Miller’s by coronally advanced flap with Pericardium® membrane.

An atypical manifestation of mucocele - A case report

Mucocele is a painless benign swelling in the oral cavity caused by a blockage of salivary gland or its duct. Although they are painless but may cause difficulty to patients for eating and speaking. Most mucoceles are visually identifiable. Only a few of mucoceles do not require any special treatment, and most of them can be removed by surgical excision. Mucoceles most commonly affect young patients. Usually, extravasation mucoceles are located in the lower lip and retention mucoceles can be seen at any site in the oral cavity. Trauma is the main causative factor involved in the formation of mucoceles. It is essential to visually recognize mucocele, for its proper treatment. This article reports a case of mucocele in a patient with sudden trauma on the lower lip. It was clinically diagnosed as traumatic fibroma because of its atypical presentation which was treated by surgical excision.
